The first stage of the Federal Government’s transition care program will be rolled out from next week.
It’s hoped the $300 million investment will free up hospital beds for people with acute illnesses and shorten hospital waiting lists.
Minister for Aging Justine Elliot says it will help older Australians recover and regain their independence after a major hospital stay.
“It could include a whole range of low intensity therapy services, or nursing support, or also personal care services,” Ms Elliot said.
“It also means older Australians won’t be going to a nursing home prematurely.
“It means they can return to their homes and receiving the appropriate care, whether it’s things like physiotherapy or occupational therapy.”
