A spokesman said the residential building site which collapsed in Sydney’s south, and killed a worker while injuring two others, wasn’t properly supported.
The three men were buried under wet concrete and steel after the second floor of the house they were building in Bundeena collapsed while the floor was being laid.
It took paramedics approximately 90 minutes to reach the man who died.
His body could not be retrieved immediately as rescue workers were forced to abandon the site because it was deemed too unstable. The body of the man had to be retrieved several hours later.
A Construction, Forestry, Mining and Energy Union spokesman said a safety investigator visited the site and the union had placed a ban on work until investigations on the incident were complete.
"Supports for the form work structure were inadequate and when several tonnes of concrete was poured into the structure it collapsed under the weight," said a CFMEU spokesman.
WorkCover NSW is making preliminary inquiries into the incident and will be in contact with the CFMEU.
